Practice Details:
•Primary Care Services: Providing comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and the elderly.
•Preventive Care: Conducting routine check-ups, health screenings, and immunizations to prevent illnesses and promote overall health and well-being.
•Diagnosis and Treatment: Diagnosing and treating common acute illnesses, such as respiratory infections, urinary tract infections, and minor injuries, as well as chronic conditions like diabetes, hypertension, thyroid disorder and asthma.
•Chronic Disease Management: Managing chronic diseases through ongoing monitoring, medication management, lifestyle counseling, and referrals to specialists when needed.
•Health Promotion and Education: Providing health education and counseling to patients on topics such as nutrition, exercise, smoking cessation, and stress management to promote healthy lifestyle choices.
•Mental Health: Promoting mental health and preventing substance use disorder by providing initial assessment, treatment and initiating referrals as appropriate.
•Referrals and Coordination of Care: Referring patients to specialists, diagnostic tests, and other healthcare services as necessary, and coordinating care among multiple providers to ensure continuity and quality of care.
•Prescription Management: Prescribing medications, reviewing medication regimens, and monitoring for potential drug interactions or adverse effects.
•Minor Procedures: Performing minor surgical procedures, such as removing staples, moles, warts or skin lesions, ear syringing and administering injections.
•Telemedicine Consultations: Providing virtual consultations and follow-up care through telemedicine platforms, especially in situations where in-person visits are not feasible or necessary.
•Electronic Health Record (EHR) Documentation: Maintaining accurate and up-to-date electronic health records of patient encounters, including medical history, physical examinations, diagnostic test results, treatment plans, and follow-up care.
